Health Insurance Open Enrollment 2027: Dates, Deadlines & What Changed

If you buy your own health insurance through the ACA Marketplace, there is an important change coming for 2027: the federal Open Enrollment period is getting shorter.

For people using HealthCare.gov, Open Enrollment for 2027 coverage will run from November 1 through December 15, 2026. Coverage selected during that period will begin January 1, 2027.

That is different from the schedule used for 2026 coverage, when federal Open Enrollment continued until January 15.

If you remember having until January to sign up in previous years, do not assume you have that extra month this time.

Important: These are the dates for Exchanges operating on the federal platform, including HealthCare.gov. If your state operates its own Marketplace, its dates may be different.

Why Did the Open Enrollment Deadline Change?

For the past several years, people using HealthCare.gov generally had from November 1 through January 15 to enroll in Marketplace coverage.

The Centers for Medicare & Medicaid Services changed the rules beginning with coverage for plan year 2027.

Under the new rules, Exchanges using the federal platform will have an Open Enrollment period running from November 1 through December 15. Plans selected during Open Enrollment will take effect January 1.

CMS said the shorter enrollment period is intended to simplify coverage start dates, encourage people to maintain continuous coverage and reduce the ability to wait until a medical need appears before buying insurance.

The practical consequence is simple: people who previously waited until January to choose a Marketplace plan may be too late for 2027 coverage.

Does Every State Have a December 15 Deadline?

No.

States that operate their own health insurance Exchanges have some flexibility to set their own Open Enrollment dates.

Beginning with plan year 2027, the federal rules generally require an Exchange’s Open Enrollment period to:

  • Begin no later than November 1
  • End no later than December 31
  • Last no more than nine weeks
  • Provide a January 1 effective date for plans selected during Open Enrollment

That means a state Marketplace may give residents more time than HealthCare.gov does, but consumers should not assume that their state’s deadline is January 15.

If you live in a state with its own Marketplace, check that Marketplace’s 2027 dates before relying on the federal December 15 deadline.

Why Do Some Websites Still Say Open Enrollment Ends January 15?

You may see conflicting information online, including pages that still describe Marketplace Open Enrollment as running from November 1 through January 15.

That was the schedule for recent coverage years, including the Open Enrollment period for 2026 coverage.

The change takes effect with plan year 2027. CMS’s final rule specifically sets the federal-platform Open Enrollment period at November 1 through December 15 beginning with 2027 coverage.

When checking an enrollment deadline, make sure the information you are reading specifically refers to 2027 coverage rather than a prior year or a general Marketplace description that has not yet been updated.

When Should You Start Shopping for a 2027 Plan?

You do not need to buy a plan on November 1, but there is little advantage to waiting until the last minute.

Once 2027 plans and prices become available, compare:

  • Your monthly premium
  • Your deductible
  • Your maximum out-of-pocket cost
  • Your doctors and hospitals
  • Your prescription drug coverage
  • Your plan’s provider network
  • Any premium tax credit or other financial assistance for which you qualify

Do You Need to Re-Enroll If You Already Have Marketplace Insurance?

Do not assume that keeping the same plan without looking at your options is the best choice.

Plans, premiums, provider networks, prescription formularies and financial assistance can change from one year to the next. Your own household income or family situation may have changed as well.

Even if automatic re-enrollment is available to you, Open Enrollment is a good time to update your information and compare your current coverage with the other 2027 plans available in your area.

A plan that was a good value in 2026 may not be the best value in 2027.

What Happens If You Miss the December 15 Deadline?

If you use HealthCare.gov and do not enroll during the 2027 Open Enrollment period, you generally cannot simply come back in January and buy a Marketplace plan because you changed your mind.

You may still be able to enroll if you qualify for a Special Enrollment Period.

Qualifying events can include situations such as:

  • Losing qualifying health coverage
  • Getting married
  • Having or adopting a child
  • Certain moves
  • Other qualifying changes in your circumstances

The rules depend on the event, and there are deadlines for using a Special Enrollment Period.

Medicaid and the Children’s Health Insurance Program, or CHIP, have different enrollment rules and generally accept applications throughout the year.

What If You Lose Job-Based Health Insurance After December 15?

Losing qualifying employer health coverage can create a Special Enrollment Period, so missing the annual Open Enrollment deadline does not necessarily mean you must remain uninsured for the entire year.

Depending on your circumstances, you may have several choices, including Marketplace coverage or continuing your former employer’s plan through COBRA.

The right comparison is usually not just the monthly premium. Look at the subsidy available through the Marketplace, deductibles, provider networks, prescription coverage and how long you expect to need the replacement insurance.

Is December 15 the Deadline for Employer Health Insurance Too?

No.

Employers set their own enrollment periods for job-based health plans. Your company’s benefits enrollment may happen in October, November, December or at another time entirely.

The December 15 deadline discussed here applies to 2027 coverage through Exchanges using the federal Marketplace platform.

Is Medicare Open Enrollment the Same Thing?

No. Medicare has separate enrollment periods.

The Medicare Annual Enrollment Period generally runs from October 15 through December 7. That is when people already in Medicare can make certain changes to Medicare Advantage and prescription drug coverage.

Marketplace Open Enrollment and Medicare Open Enrollment sound similar but apply to different types of insurance and different groups of people.

What Should You Do Before November 1?

You do not have to wait until Open Enrollment begins to get organized.

Before November, gather:

  • Your best estimate of household income for 2027
  • Information about everyone who needs coverage
  • A list of doctors and hospitals you want to keep using
  • Your regular prescription medications
  • Your current plan information
  • Any notices you receive about renewal or changes to your current coverage

Having this information ready makes it much easier to compare plans once 2027 options are available.
